When Is a Tooth Extraction Necessary?

There are a number of situations in which extracting a tooth is the most appropriate course of action. The most common reasons we recommend extractions at Eastern Family Dental include:

Severe Tooth Decay When decay has progressed so deeply into a tooth that not enough healthy structure remains to support a crown or filling — and a root canal is not a viable option — extraction is often the only way to eliminate the source of infection and protect the surrounding teeth and bone.

Advanced Gum Disease Periodontal disease that has progressed significantly can cause the bone and tissue supporting a tooth to deteriorate to the point where the tooth becomes loose and cannot be stabilized. In these cases, extraction removes the compromised tooth and allows the surrounding tissue to heal.

Cracked or Fractured Teeth A tooth that has fractured below the gumline or suffered a crack severe enough that it cannot be restored may need to be extracted to prevent infection and further damage to the surrounding structures.

Dental Abscess or Infection An abscess is a pocket of infection that develops at the root of a tooth or in the surrounding gum tissue. When the infection is too advanced to be resolved with root canal therapy or antibiotics alone, extraction eliminates the source of infection and prevents it from spreading.

Orthodontic Treatment In some cases, tooth extraction is a planned part of orthodontic treatment. When the jaw is too crowded to allow teeth to shift into proper alignment, removing one or more teeth creates the space needed for successful treatment.

Impacted Wisdom Teeth Wisdom teeth — the third molars at the back of the mouth — frequently do not have enough room to erupt fully or properly. Impacted wisdom teeth can cause pain, crowding, infection, and damage to adjacent teeth. Removal is often the recommended course of action before problems develop or worsen.

Baby Teeth That Won’t Fall Out Occasionally, primary teeth do not fall out on their own schedule and begin to interfere with the eruption of permanent teeth beneath them. Extracting a stubborn baby tooth creates the path a permanent tooth needs to come in correctly.

Types of Extractions We Perform

Simple Extractions

A simple extraction is performed on a tooth that is fully visible above the gumline and can be loosened and removed with dental instruments without the need for incisions. Simple extractions are performed under local anesthesia and are typically quick and straightforward procedures.

Most patients are surprised by how manageable a simple extraction is. With proper anesthesia, you should feel pressure during the procedure but not pain.

Surgical Extractions

A surgical extraction is required when a tooth cannot be accessed or removed as straightforwardly — most commonly because it is impacted beneath the gumline, has broken off at the gumline, has curved or unusually shaped roots, or has not fully erupted.

Surgical extractions involve a small incision in the gum tissue to access the tooth. In some cases, the tooth may be divided into sections for easier removal. Surgical extractions are performed under local anesthesia, and sedation options are available for patients who experience dental anxiety.

Wisdom Tooth Extractions

Wisdom tooth removal is one of the most common oral surgery procedures we perform. Whether your wisdom teeth are fully impacted, partially erupted, or causing crowding and discomfort, our team will evaluate their position and recommend the most appropriate approach.

We recommend wisdom tooth evaluations for teenagers and young adults before problems have a chance to develop — proactive removal is almost always easier and heals faster than waiting until symptoms appear.

What to Expect During Your Extraction Appointment

We want you to feel informed and at ease before you arrive. Here is a general overview of what your extraction appointment at Eastern Family Dental will look like:

Pre-Procedure Evaluation Before proceeding, we will review your dental X-rays, examine the tooth and surrounding tissue, and confirm our treatment recommendation. This is a great time to ask any questions you have about the procedure, recovery, or tooth replacement options.

Anesthesia Local anesthesia is administered to thoroughly numb the tooth, surrounding gum tissue, and jawbone in the treatment area. We take our time with this step — ensuring you are completely numb before we proceed is non-negotiable. You will feel pressure and movement during the extraction but should not experience pain. If at any point you feel discomfort, let us know immediately.

The Extraction For simple extractions, the tooth is gently loosened using a dental instrument called an elevator and then removed with forceps. For surgical extractions, a small incision is made, and the tooth is carefully accessed and removed — sometimes in sections. The entire procedure is typically faster than most patients expect.

Gauze and Initial Clot Formation Once the tooth is removed, gauze is placed over the socket and you will be asked to bite down gently to apply pressure. This helps a blood clot form in the socket — a critical first step in healing.

Post-Procedure Instructions Before you leave, we will walk you through detailed aftercare instructions and answer any questions about your recovery. Following these instructions carefully makes a significant difference in how smoothly you heal.

Extraction Aftercare: What to Do at Home

Proper aftercare in the days following your extraction helps ensure comfortable, complication-free healing. Here is what we recommend:

The First 24 Hours

  • Keep the gauze pad in place and bite firmly for 30 to 45 minutes after the procedure to support clot formation
  • Avoid rinsing, spitting forcefully, or using a straw — suction and pressure can dislodge the clot
  • Apply an ice pack to the outside of your cheek in 20-minute intervals to minimize swelling
  • Rest and avoid strenuous physical activity for the remainder of the day
  • Take any prescribed medications as directed and over-the-counter pain relievers as needed

The First Few Days

  • Eat soft foods — yogurt, eggs, mashed potatoes, smoothies, soup — and avoid chewing near the extraction site
  • Avoid hot, spicy, or crunchy foods that could irritate the socket
  • Do not smoke or use tobacco products, as this significantly increases the risk of dry socket and slows healing
  • Continue brushing and flossing your other teeth, but avoid the extraction site directly
  • After 24 hours, gently rinse with warm salt water after meals to keep the area clean

When to Call Us Contact our office if you experience any of the following after your extraction:

  • Severe or worsening pain after the first two to three days
  • Bleeding that does not subside with gauze pressure after an hour
  • Signs of infection including fever, increased swelling, or discharge
  • A dry, aching sensation deep in the socket that may indicate dry socket

What Is Dry Socket — and How Do You Avoid It?

Dry socket is one of the most common complications following a tooth extraction. It occurs when the blood clot that forms in the empty socket becomes dislodged or dissolves before the socket has had a chance to heal — exposing the underlying bone and nerves.

Dry socket typically presents as a dull, throbbing ache that radiates into the jaw or ear, often beginning two to three days after the extraction. It is uncomfortable but very treatable — contact our office and we will have you in promptly.

The best way to prevent dry socket is to follow your aftercare instructions carefully, avoid smoking, and resist the urge to use a straw or rinse vigorously in the first 24 hours.

Tooth Replacement After Extraction

Extracting a tooth solves an immediate problem — but the story doesn’t end there. When a permanent tooth is removed, the surrounding teeth will naturally begin to drift toward the empty space over time. This can affect your bite, your ability to chew, and the alignment of your remaining teeth.

We strongly encourage patients to discuss tooth replacement options at the time of extraction — or shortly after healing is complete. At Eastern Family Dental, we offer several restorative options to fill the space left behind, including:

  • Dental implants — the most durable, natural-feeling tooth replacement option available
  • Dental bridges — a fixed restoration anchored to the teeth on either side of the gap
  • Partial dentures — a removable option for replacing one or more missing teeth

Frequently Asked Questions About Tooth Extractions

Will I be awake during my extraction?

Yes, most extractions are performed under local anesthesia while you are fully awake. You will feel pressure and movement but should not feel pain. If dental anxiety is a concern, talk to us about sedation options that can help you feel calm and relaxed throughout the procedure.

How long does recovery take?

Most patients feel significantly better within two to three days. Complete tissue healing typically takes one to two weeks, though the underlying bone continues to remodel over several months.

Can I go back to work after an extraction?

Many patients return to desk work or light activity the day after a simple extraction. We recommend taking the remainder of the extraction day to rest. For surgical extractions, a day or two of downtime is advisable.

What if I am on blood thinners or other medications?

Always inform our team of all medications and supplements you are taking before your procedure. Certain medications can affect bleeding and healing, and we will coordinate with your physician if any adjustments are needed before your extraction.

How soon can I get a replacement tooth after extraction?

The timeline depends on which replacement option you choose. Dental implants typically require a healing period of several months before placement. Bridges and partial dentures can often be placed sooner. We will discuss timing and next steps at your consultation.
